UNIFORM HAZARDOUS WASTE MANIFEST

If you are shipping hazardous waste, you must send it with a Uniform Hazardous Waste Manifest. This sample manifest can be found in the Appendix to 40 CFR Part 262 of the federal hazardous waste regulations. Keep in mind that the actual manifest form is six pages and must be obtained and distributed according to the requirements in §262.21–§262.23.

RCRA SUBTITLE C SITE IDENTIFICATION FORM

Need an EPA ID Number? If you manage hazardous waste, you probably do. And the RCRA Subtitle C Site Identification Form (EPA Form 8700-12) is the form you must complete and submit when you make your initial notification—and for any subsequent revisions. It’s also used for some of the new notifications under the recent generator improvements rule.

MASTER LDR FORM

Required notifications and certifications under the land disposal restrictions (LDR) can often be satisfied using forms from your TSD facility. But what about the notifications or certifications that do not involve offsite facilities? Forms for these purposes can be difficult to find, so we have created a form you can use to meet the LDR paperwork requirements in these cases. Download form >

LEAD-ACID BATTERY LDR FORM

This LDR form can be used by people that generate, collect, and/or transport lead-acid batteries destined for reclamation under §266.80. Because the batteries are being shipped offsite and don’t meet the appropriate treatment standard, the notification in §268.7(a)(2) is required. This form can be used to satisfy that requirement. LDR ONE-TIME NOTICE

As a general rule, you should have a §268.7(a)(7) one-time notice on file for every hazardous waste you generate if the waste subsequently is excluded or exempted from regulation. We created this simple form to help you comply with the LDR one-time notice requirement. Although the master form may also be used for this purpose, this simplified form has been streamlined specifically for §268.7(a)(7) one-time notices. Download form >

LAB PACK LDR FORM

Lab packs containing hazardous wastes that will be incinerated under the alternative LDR treatment standards in §268.42(c) require a special notification/certification. This form accommodates the information requirements specified in §268.7(a)(9) of the generator paperwork requirements. Download form >
